Membership How to Join
The SGMP membership application is available here as a PDF (Adobe Acrobat) file. You may download this file, then print it locally from your computer. You need to have Acrobat Reader to view and print this file. This software is available free of charge. There is also a Microsoft Word version if you do not have Adobe Acrobat Reader, but we would prefer you use the PDF file.

Complete the printed application. Planners AND CONTRACT PLANNERS must also complete and return the planner profile with the application. If you wish to affiliate with a chapter, MAIL your ORIGINAL SIGNED AND DATED application, along with your membership fee payable to SGMP, to the membership contact listed for the chapter you wish to affiliate with. There is a $40.00 return check fee.
If you wish to join as a member-at-large, or there is no membership contact listed for the chapter you wish to join, then MAIL YOUR ORIGINAL SIGNED AND DATED APPLICATION AND PAYMENT TO:

Faxed or emailed applications WILL NOT BE ACCEPTED and will delay the processing or your membership!
Note: Supplier applications must be submitted with a completed planner application or they will be returned to the applicant.
