SGMP membership applications are available here as a PDF (Adobe Acrobat) files. You may download the appropriate file, and type and select appropriate responses. You need to have Acrobat Reader to view, edit and print this file. This software is available free of charge.

If you are not sure which membership category you are in, please refer to our membership category definitions.
Complete the printed application. If you wish to affiliate with a chapter, MAIL your ORIGINAL SIGNED AND DATED application, along with your membership fee payable to SGMP, to our headquarters address below. There is a $40.00 return check fee.

Note: Supplier applications must be submitted with a completed planner application or they will be returned to the applicant. Suppliers are required to join the same chapter as their planner match. Supplier applicants for the Arizona, Buckeye, Crossroads of America, Heart of America, Heart of Illinois, Old Dominion, and Pacific Northwest chapters currently must submit two planner matches in the same month in order to join SGMP.
